You may reach us at 574-534-9911. Our fax number is 574-534-6915. We are in the office Monday, Tuesday, and Wednesday from 8:00 am to 5:00 pm; and Thursday and Friday from 8:00 am to 12:00 noon. We are closed from 12:00 noon to 1:00 pm for lunch.

We strive continually to minimize Billing Headaches for our patients!

The billing department is run by Brenda Hochstetler.

We accept all insurance, and are members of most large insurance plans such as Sagamore, Anthem Blue Cross Blue Sheild. We also accept assignment from Medicare and Medicaid.

All of our billing is done in the office - it is not sent out to another company for processing - this enables us to have complete control and knowledge of your account. We encourage an open relationship between our billing department and our patients.
